Nursing Home Without Walls
Nursing Home Without Walls
York Care Centre is proud to be a part of the Nursing Home Without Walls (NHWW) program, a transformative initiative designed to enhance the aging experience. The NHWW program empowers older adults and their caregivers in New Brunswick to age in their own homes.
Created in 2019 by Dr. Suzanne Dupuis-Blanchard from the Université de Moncton, in collaboration with Social Development and Healthcare Excellence Canada, NHWW focuses on facilitating healthy aging at home. The program provides access to essential knowledge, support, and services, enabling older adults to maintain their independence and well-being.
Support and Services
How can we help?
Nursing Home Without Walls offers comprehensive support to older adults aged 65+ living on Fredericton’s Northside, aiming to enhance their quality of life while they continue to live at home. Our program provides a variety of essential services, including:
Navigation of NB Health Services and Applications: Assistance with understanding and accessing health services and completing necessary applications.
Social Engagement: Opportunities to connect with others through various social activities and events.
One-on-One Wellness Visits: Personalized wellness visits to monitor and support individual health and well-being.
Telephone Check-Ins: Regular phone calls to check in on well-being and provide companionship.
Education Sessions: Informative sessions on various topics relevant to aging and health.
Our goal is to empower older adults and their caregivers by providing the support and services needed to live well at home.
